From I-95: Take Exit 329 for County Road 210. Proceed west on 210 towards Green Cove Springs. 

After 1.0 mile, you will pass a Regions Bank on your right.  You will then immediately go onto the left turn lane to  take a left onto Kingsley Lake Drive.

You will enter Southlake Office Center.  The road will curve to the right. 

Our office is located in the last building on your left. 

We are in Suite 402 and share office space with Beaches Dermatology and the Family Foot & Ankle Center - Dr. John Lagoutaris.

From CR-13: Travel east on CR-210.  Take a right onto Prosperity Lake Drive.  Take your first left onto Kingsley Lake Drive.  We are located in the first building on your left.
