From I-95 North: Take Exit 335 for Old St. Augustine Road.  Turn right onto Old St. Augustine Road and continue for 0.7 miles.  Our office is located on the right  just after the YMCA building on the right. 

We are in a two story brick building.  Other practices in the building include First Coast Cardiovascular Institute, Bowden Eye Associates, Baptist Pediatrics, and Memorial Family Practice. 

We are in Suite 201.  Please proceed to the entrance facing Old St. Augustine Road.
